I hereby release and discharge Elite Performance and Strength, its owners, employees, and representatives from any and all liability, claims, or damages that may arise from my use of the facility or participation in open gym activities, including but not limited to injuries or damages sustained while using the facility’s equipment.

I agree to follow all safety instructions and posted rules. I acknowledge the facility’s right to deny access or participation if I fail to comply with the rules or am deemed unfit to participate. If I am unfamiliar with any equipment, I will seek proper guidance before use.

I confirm I am physically capable of participating in open gym activities and have no known medical conditions or disabilities that would interfere with my ability to exercise. I understand it is my responsibility to consult a healthcare provider if I have health concerns.

I agree to indemnify and hold harmless Elite Performance and Strength, its owners, employees, and representatives from any claims, demands, actions, or costs arising from my participation in open gym activities, including any injury or damage caused to myself or others due to my negligence.

I acknowledge that I have read and fully understand this waiver and release form, and I voluntarily accept the risks and responsibilities involved in participating in open gym activities. By signing, I waive my right to seek legal action or compensation for any injuries or losses.
